The reason for the “false negative” findings of liver ultrasound is the small size of metastases. So, in our observations, out of 14 cases with secondary foci of 11 that were not detected before the operation, they had sizes less than 1.5 cm. According to this method, the sensitivity of the method is 80.5% for metastases greater than 1.5 cm, and up to 1 cm for metastases. – Only only 37%. In addition, the visualization of metastases with the echodensity of the hepatic parenchyma becomes possible only after the development of peripheral edema around
tumor tissue and as a consequence of the appearance on the echograms “mourning border”. In observations, undetected metastases were more often located in the right lobe under the diaphragm.
Despite the presence of false-negative and false-positive results, in general, the accuracy of ultrasound diagnosis of lung cancer metastases to the liver is quite high and, according to our data, was 87.7%, and the presence of metastases was confirmed in 81.8% of cases, and the absence was in 89.2%. The sensitivity of the method was 65.8%, specificity —95.1%.
Ultrasonic diagnosis of lung cancer metastases in the kidneys and adrenal glands is in our opinion less informative. Thus, with 151 pathologic examinations of the corpses of deceased patients, secondary foci in the kidneys were detected in 17 observations, in the adrenal glands — at 16, while it was possible to visualize them echo-tomographically in 7 and 5 cases. All conclusions about the malignant nature of the detected nodes in the kidneys and adrenal glands coincided with the autopsy data, but dynamic observation of discharge lung cancer patients with identified changes in these organs shows that at this stage of development of ultrasonic diagnostic equipment, the use of echography does not allow to unequivocally establish their nature and requires conducting a diagnostic diagnosis up to the morphological verification of the process,if its results can significantly influence the treatment tactics.
